CALIFORNIA DEPARTMENT OF EDUCATION SPORTING EVENT/GAME INCIDENT REPORT

This form is to document incidents of racial discrimination, harassment, and/or hazing that occur at high school sporting games or sporting events. Anyone who experiences, witnesses, or becomes aware of such behavior is encouraged to complete and submit this form to the District Office Student Services as soon as possible after the incident. You may also report a hate incident or hate crime to the California Civil Rights Department’s CA vs. Hate Resource Line and Network. Reports can be made online or by calling (833) 866-4283, Monday to Friday from 9 a.m. to 6 p.m.
